Sagittal appliance를 이용한 부정교합의 치료에 관한 증례

Abstract
결론
저자들은 전남대학교 병원 소아치과에 내원한 반대교합, 심한 총생을 갖는 부정교합과
submerged tooth로 인한 제 1대구치의 근심 경사를 갖는 환아를 Sagittal appliance로 치료
하여 다음과 같은 결론을 얻었다.
1. 상실된 악궁 길이로 인한 총생은 제 2대구치를 발치하여 대부분의 구치부 후방이동과
약간의 절치부 전방이동으로 해소되었다.
2. Premaxilla의 저성장으로 인한 반대교합은 제2대치를 발치하지 않고 premaxilla와 절치
를 전방 이동시킴으로써 해소되었고 또한 안모의 개선을 보였다.
3. 장치를 쓰는 과정에서 발생되는 불규칙한 치아의 배열과 절치의 순측경사는 고정성 교
정장치를 이용해야 될 것으로 사료된다.
4. Submerged된 하악 좌측 제2유구치로 인해 근심 경사된 제 1대구치는 편측 sagittal
appliance를 이용하여 근심경사를 바로잡고 상실된 제2유구치 공간을 확보했다.

Sagittal appliance is an active plate with expansion screws in the anteroposterior
direction. It is used to advance the maxillary incisors and to distalize the maxillary
molars, thereby to increase arch length.
Sagittal appliance produces changes of tooth position in the surrounding bone by
convertion mechanical pressure into biological energy as the screws are activated
regularly.
We treated anterior crossbite, severe corwding, and submerged tooth with sagittal
appliance.
The result were as follows :
1. Dental crowding was relieved by some advancement of incisors and most of
distalization of molars after extraction of second molars.
2. Anterior cross bits caused by immaturity of premaxilla was corrected by
advancement of maxillary incisor and premaxilla and then facial profile was improved.
3. Irregular tooth arrangement and labial inclination of incisors occured during
treatment will be corrected by fixed appliance.
4. mesially titled first molar caused by submerged second primary moalr was
uprighted and lost space of second primary molar was regained by lower unilateral
sagittal appliance.
